If you have an insurance that is not on this list, and you have Out-of-Network Benefits and are willing to pay as Out-of-Network, then you may become a patient upon approval from your insurance. Please contact us for more information. Certain conditions or restrictions may apply.

Sliding Fee Scale: No one will be denied access to services due to inability to pay; and there is a Discounted Fee Scale available upon request.


For Established Patients: If your insurance changes to one not on this list, to continue services at CBH, you will have to either be approved by your insurance for Out-of-Network services or will assume the cost of services (self pay).
